1.  Please download and complete the Polaris order form by selecting the model you are interested in, note any accessories you want quoted and complete the bottom portion.  Please fax the form to 763-417-2145 or e-mail it to sheila.shaughnessy@polarisind.com.

2.  An official quote will be returned to you.

3.  Make any changes you want and send back to contact.  Then when finalized, send the purchase order via fax to 763-417-2145 or e-mail to sheila.shaughnessy@polarisind.com.
